Digital Innovation Mapping in Health & Social Care

The London Office of Technology & Innovation (LOTI) is running the Adult Social Care Sandbox, which has engaged with Health & Social Care professionals across the capital to identify the pain points and opportunities for solutions – for both staff and people needing care – within the system.

The Sandbox has been really well received and has featured a series of immersive events, workshops and design sessions. A key asset of the programme is LOTI’s Health & Social Care System Map, which gives a valuable visual framing of the current system and challenges (see map below). 

One of the next stages of the Sandbox is to collate the latest information on the digital solutions which have been designed to support the delivery of – and access to – Health & Adult Social Care services. CC2i has been commissioned by LOTI to research and identify the solutions that meet the outlined pain points. We will then overlay the digital and data solutions onto the map, to help LOTI and their members to understand how they can go about addressing and solving the challenges.

Via this form we are inviting organisations to give a brief overview of their digital & data products, so we can map them onto the existing Health & Social Care System Map. The solutions identified through this process will be shared with LOTI boroughs and their partners.

The form should only take 10 minutes to complete, and there are a couple of key instructions to follow:

  • Any digital / data solution submitted must be ready for deployment into health & social care setting from Jan 2025;
  • Any digital / data solution submitted must address one of the following key challenge areas:
    • Community Interventions – specifically exploring how we can support people with recent diagnoses and prevent unnecessary hospital admissions through innovative community-based solutions;
    • Transfer of Care – in particular technology that can improve the process for both patients and staff, ensuring smoother transitions, better information sharing, and appropriate support;
    • Local Authority Needs and Financial Care Assessments and how we can make these processes more responsive, user-friendly, and accessible for residents and carers;
    • Triaging of Messages in Local Authorities – solutions that streamline communication and referral processes to help social care teams respond more efficiently;
    • Additional ASC Pain Points:
      • Improving access to ASC Information & Advice
      • Supporting Unpaid Carers
      • Reducing the escalation of health issues
      • Avoiding hospital admissions
      • Addressing frailty from hospital stay
      • Addressing the complex pre-admission assessment process
