Hospital to Home Presentation and Demonstration
Our Festival of Health and Social Care 2021 ran on the 1st October, and was a fantastic event with registration and attendance from over 250 people from over 75 local authorities and relevant health, care and technical partners. The day included thought provoking discussions from our councils involved in CC2i’s collaborations over the past 18 months and informative demonstrations and presentations from our technical partners.
We had a section of the morning dedicated to our Adult Social Care Innovation Focus, involving 20-minute slots where technical partners gave an overview of what they had developed and why – they then moved on to lunchtime demonstrations of the platforms.
Hospital to Home is a digital solution that removes the complexity around discharge to assess and it ensures patient safety through improved D2A management. Currently, there is no one source of truth around a person in the discharge to assess process. So much time and effort is given to phone calls, emails, spreadsheets, to even physically being on wards asking questions to try and keep track of discharges and following them through to their assessment at home. Hospital to Home cuts through all of that by providing the one source of truth. At any time you can see an up to date view of all your discharges, the teams and people who are involved and the pathways which your discharges are on. This is all supported by an extensive reporting suite and robust audit and governance management.
